Intervenciones personalizadas de cuidado enfermero en una persona con enfermedad de hipertensión arterial sistémica y obesidad grado-I

Abstract

Non-communicable diseases are a public health problem because they are the leading cause of death in the world, systemic arterial hypertension and uncontrolled grade-I obesity could lead to cardiac and kidney complications or some retinopathy. Nursing highlights, the importance of its actions through the theoretical-methodological support of the nursing process, when identifying and solving real or potential health problems of the individual, family and community. The care plans are a tool that allows improving the nursing practice and the quality of life of the user. Objective: To implement a care plan with personalized interventions for a person with Systemic Arterial Hypertension and Grade I Obesity disease. Method: A documentary review of the pathophysiological process of systemic arterial hypertension and grade-I obesity was carried out to support the study condition and elaborate elements that make up this thesis; The data collection was carried out under the guidance of evaluation of the process with the reference of Dorothea E. Orem, permeating the theoretical-methodological support of the nursing process, followed by an analysis, selection and prioritization of diagnoses, the planning of interventions, actions and evaluation supported under NANDA-I, NOC and NIC taxonomies. Results: an individualized nursing care plan was developed based on 5 nursing diagnoses, 5 results, 14 indicators, 6 interventions and 75 nursing activities to resolve the diagnoses and improve their health. Conclusion: the analysis of the results shows the magnitude and importance of the Nursing Care implemented in a person with systemic arterial hypertension and grade-I obesity, which allows reaffirming the need to provide comprehensive nursing care to favor the person.

Las enfermedades no transmisibles son un problema de salud pública por ser la primera causa de muerte en el mundo, la hipertensión arterial sistémica y la obesidad grado-I sin control, podrían llevar a complicaciones cardiacas, renales o alguna retinopatía. Enfermería resalta la importancia de su actuar mediante el soporte teórico-metodológico del proceso enfermero, al identificar y resolver problemas de salud reales o potenciales del individuo, familia y comunidad. Los planes de cuidados resultan una herramienta que permite mejorar la práctica de enfermería y la calidad de vida del usuario. Objetivo: Implementar un plan de cuidados con intervenciones personalizadas a una persona con enfermedad de Hipertensión Arterial Sistémica y Obesidad grado-I. Método: Se realizó una revisión documental del proceso fisiopatológico de la hipertensión arterial sistémica y la obesidad grado-I para sustentar la condición de estudio y elaborar elementos que conforman esta tesis; la colecta de datos se realizó bajo la guía de valoración del proceso con el referente de Dorothea E. Orem, permeando el sustento teórico-metodológico del proceso enfermero, enseguida un análisis, selección y priorización de diagnósticos, la planeación de intervenciones, acciones y evaluación sustentado bajo taxonomías NANDA-I, NOC y NIC. Resultados: se elaboró un plan de cuidados individualizado del cuidado enfermero basado en 5 diagnósticos de enfermería, 5 resultados, 14 indicadores, 6 intervenciones y 75 actividades de enfermería para resolver los diagnósticos y mejorar su salud. Conclusión: al análisis de los resultados se observa la magnitud y trascendencia que tienen los Cuidados de Enfermería implementados en una persona con hipertensión arterial sistémica y obesidad grado-I, lo que permite reafirmar la necesidad de brindar cuidado integral de enfermería para favorecer a la persona.
